De-Institutionalization in Upper Austria: Problem solving or only Trans-Institutionalization?

In the Sociology of mental health deinstitutionalization of former psychiatric long-stay patients is often criticised as transinstitutionalization. A prospective cohort study of a randomized sample (n=116) of all long stay patients (n=409) discharged between 10.10.1995 and 31.12.1999 from the central mental hospital of Upper-Austria evaluates results of the deinstitutionalization process. The study ask, how former mental long-stay patients do assess their quality of life, which services of the social psychiatric community network they use, if needs are met and what direct costs are connected with the utilization. It compares the direct costs of the new care system with former costs in the mental hospital. At first view results denote, that the decision for deinstitutionalization has been successful and means a win-win-situation: former patients as well as the public profits from this community based model not only in terms of lower costs. But considering deinstitutionalization has been conducted more or less “institutional oriented” and has included most elderly patients, problems like how to organize good care for “new long stay patients” or homeless mentally ill still persist.
